What is the cost of the session?
In-person or virtual evaluation: $250
In-person or virtual follow up visit: $200
15 minute consult- Free
How long are sessions?
All sessions whether in person or online are 60 minutes in length except 15 minute phone consults.
Do you take insurance?
No, but your insurance provider might be able to help you cover the cost. Physical Therapy is provided on a “fee at time of service” AKA “cash pay” basis and is considered out of network. Once your visit is booked, your credit card is placed on file and is charged after your visit. After your visit a superbill can be provided upon request and submitted by you to insurance for reimbursement of services provided. Depending on your insurance and whether or not your deductible has been met, you may receive reimbursement. You can always call your insurance company to find out if you will be reimbursed for out of network Physical Therapy. Because your visit doesn’t go through insurance there is no limitation in treatment time, treatment provided, number of visits we can have, or wait time to receive care. As a result you will have longer skilled one on one time with me for evaluation and treatment, have better outcomes in fewer overall visits, and customized care delivered to your door.
Do I need a referral from my MD?
No! Oregon is a direct access state which means you don’t need a referral to see a physical therapist! However, if you want to get one or have one already please send it or provide me a copy.
May I use my FSA or HSA for payment?
Yes, you can use your FSA or HSA card, once the payment has been processed you will receive an emailed receipt. You can use this receipt to submit for reimbursement with your FSA or HSA company.
